Summer Program

Be Healthy and Physically Active!

Come join us for lots of interactive learning and physical activities - Summer 2013

Who wants to feel like a million bucks? This summer at Lovely Lane Summer Daycare, we will focus on developing healthy habits for healthy kids. We will be focusing on our bodies and our health as we engage in a multitude of speakers, field trips, projects and activities. There will be lots of opportunities for physical exercise, both leisure and recreational as well as aerobic. We will also learn about healthy habits such as colorful nutritious foods, good hygiene, and drug/tobacco prevention. We will be busy doing fun things like ice skating, roller skating, fishing, bowling, miniature golf, and swimming. We will visit places like Playstation, Sport Zone, IndianCreek Nature Center and more!

Registration begins March 12th. Please bring in a completed registration form, summer contract, and the registration fee so you don't miss out on the opportunity for an active summer! If you have any questions, please call Mrs. Barnes at 393-8189.

Summer DHS Forms

Registration for our summer program will soon begin. In addition to the registration form, contract, and registration fee, you also need to fill out the required DHS forms. These forms are below and are Microsoft Word forms so you can fill them out on the computer, print them off, and then sign them and bring them in. If you prefer to fill out the paper copies, they can be picked up from Mrs. Barnes' office. PLEASE DATE THESE FORMS JUNE 1ST OR LATER SO THAT THEY CAN BE USED FOR THE NEXT SCHOOL YEAR AS WELL. Thank you!

Pick-up Authorization

Travel Authorization

Enrollment Agreement

Intake Sheet

Parental Emergency Medical Consent

Sunscreen Permission

Parent Handbook Agreement (to be filled out after reading the Parent Handbook)

Emergency Card

Immunization Form must be printed off and brought in from your doctor's office as well.
