In addition to the registration form, contract/calendar and registration fee, you also need to fill out the required DHS forms. These forms are below and are Microsoft Word forms so you can fill them out on the computer, print them off, and then sign them and bring them in. If you prefer to fill out the paper copies, they can be picked up from Mrs. Barnes’ office. PLEASE DATE THESE FORMS JUNE 1ST OR LATER SO THAT THEY CAN BE USED FOR THE NEXT SCHOOL YEAR AS WELL.
