If you have already registered with Mrs. Barnes or Ms. Dawn to attend preschool next year, your next step is filling out all of these DHS-required forms. These forms MUST be filled out, signed, and turned in before your child’s first day of preschool.
PLEASE DATE THESE FORMS JUNE 1ST OR LATER SO THAT THEY CAN BE USED FOR THE ENTIRE SCHOOL YEAR.
